Towns or small cities usually have all the necessary medical services, often better schools than big city, theaters, most everything except opera houses and places like that. Small towns are usually, but not always, close enough to a large or large enough city to have all the necessary services. Perhaps not as convenient, but in some cases, more easily accessible. Most people who move to a small city (100,000 or 200,000 population) can't believe they wasted so much of their life in the big city.
